hire a Family Medicine Physician to work in Brawley , CA, 2 hours

Family Medicine - Board Certified/Board Eligible - Ideal candidate
will have strong work ethic, outstanding clinical skills and strong
desire to serve the medically underserved - 18-22 patient/day -
Monday –Friday 8 hour shift. No weekends and no on call - We
currently have two midlevel FM we also have Peds, WH, BH that work
at least 1 to 2 days per week. - Ideal candidate will have a strong
work ethic, outstanding clinical skills and strong desire to serve
the medically underserved patient population - Provides highest
standard of outpatient medical care to patients. Including
examination, diagnosis, treatment and rehabilitation. -
Responsibility for all his activities. Including accurate charting,
good documentation of history, physical findings and assessment and
treatment plans; and keeps track of all seriously ill patients this
physician sees. - Responsible for proper case management for
patients with chronic conditions. - Arranges timely consultations
with specialists when needed by the patients. - Knowledge and
implementation of community-based, patient-centered medical home
(PCMH) process with appropriate coordination of care. Profile
