What You'll Do Conduct initial and ongoing occupational therapy evaluations in the home Assess patient functional status, including strength, endurance, coordination, communication, and self-care abilities Evaluate the home environment for safety risks and barriers to independent living Develop, implement, and update individualized plans of care in collaboration with the Health Care Provider and RN Complete initial evaluations, treatment notes, and re-evaluations at least every 30 days Complete comprehensive assessments and OASIS documentation accurately and on time Observe, document, and report patient progress and significant changes; obtain updated orders as needed Identify and recommend adaptive equipment and strategies to improve independence with activities of daily living (ADLs) Educate patients and caregivers on adaptive techniques and devices Provide supervision and support to Occupational Therapy Assistants (OTAs), as assigned Collaborate with physicians and interdisciplinary team members to deliver high-quality care Who You Are A compassionate, patient-focused Occupational Therapist Comfortable working independently in a home health setting Organized, detail-oriented, and timely with documentation A collaborative team player committed to quality outcomes Motivated by meaningful, one-on-one patient care Qualifications Bachelor’s degree from an accredited Occupational Therapy program Active Occupational Therapist license in the state of practice Minimum of one (1) year of OT experience; home health experience preferred Valid driver’s license, reliable transportation, and auto insurance Ability to complete home visits and document care accurately and timely Home health documentation experience preferred (Home Care Home Base a plus) Pay & Perks Competitive pay that reflects your experience and expertise Full-time benefits (23 days of PTO, medical, dental, vision, and disability coverage) available to full-time associates Flexible scheduling that supports work-life balance Mileage reimbursement for patient-related travel Continuing education and tuition assistance Generous referral bonus program OASIS Virtual Assistant to streamline documentation Supportive, accessible leadership team Our Mission & Values Our mission is to deliver exceptional home health care through clinical excellence, compassion, and integrity, while making a meaningful impact in the communities we serve.
