Job Description Job Description Meals on Wheels Orange County is
the largest nonprofit provider of nutrition and supportive services
for at-risk older adults in Central and North Orange County. Our
mission is to combat hunger and loneliness among older adults
through nutritious meals, friendly visits, and safety checks, and
to support families through day services. We deliver nearly 2
million meals annually to 20,000 at-risk older adults. Location:
Anaheim, CA Hours: Monday - Friday, 7:00 AM - 3:30 PM Hourly Range:
$23.00 - $26.00 per hour (Hourly rate will be determined based on
factors such as geographic location, skills, education, and
experience). JOB SUMMARY: Under the direction of the Medical Social
Worker and Program Director, the Assistant Social Worker is
responsible for identifying and assessing potential participants
for Adult Day Health Care (ADHC) admission. Acts as an advocate for
participants and their families to help solve medical social
problems. ESSENTIAL DUTIES AND RESPONSIBILITIES: Meets with
potential participants, their families or representatives to
provide facility tours and information on attending the ADHC.
Assists with the initial assessments, including home evaluations,
for potential ADHC participants. Meets with participant, family and
legal representative to complete all paperwork required for
attendance the ADHC Center. Assists with participant's
re-assessment of psychosocial needs and participants in team
development of care plan. Assists in revising care plans. Provides
counseling to participants and families in individual and group
formats. Documents, signs, and dates services provided to
participants on the day performed. Assists with the preparation of
quarterly progress reports. Provides referrals for persons not
appropriate for ADHC services. Serves as liaison with participant's
family, home, and community agencies. Assists with discharge
planning for all participants. Member of the Multidisciplinary
Assessment Team. Attends professional and community meetings as
appropriate. Performs other duties as needed. EDUCATION, EXPERIENCE
AND QUALIFICATIONS: Bachelor's degree from an accredited school in
social sciences or related field. One year’s experience working
with elderly and/or functionally impaired preferred. Must possess
excellent communication skills and good problem-solving skills.
Ability to identify participant/family needs and refer to
appropriate resources. Must be able to work as a team member. Must
pass a post-offer physical examination and obtain a tuberculosis
clearance. Must have a valid California driver's license and valid
automobile insurance. PHYSICAL JOB REQUIREMENTS AND WORK
ENVIRONMENT: This job operates in a fast-paced, multi-tasking,
professional environment, requiring ongoing learning and complex
problem-solving. Ability to sit in a stationary position for
extended periods and use both hands and fingers for typing on a
keyboard. Ability to use office equipment such as a copy machine,
printer, filing cabinets, and telephones. Occasionally must be able
to move and lift office equipment and materials to 10 pounds. Close
vision requirements due to frequent computer use. The noise level
in the work environment is usually moderate (i.e., telephones,
computers, printers, etc.). Occasional overtime and local travel
